If you are unsure how to use our script, please read the following or contact your account manager.
You will need to create a header, a footer and a stylesheet template file.
Creating your template files is relatively simple.
You can take the html/css from an existing web page of yours, removing the page content and clearing an area for the form.
Once you have this blank page ready, you then need to separate the 'header' from the 'footer'. This simply means select all the text and code from below where the form will be, in the HTML, and create your footer file from this. The remaining text, from above where the form will be, in the HTML is now your header.

If you wish to add some formatting to the form itself, use the guide above, listing the css class names, and you can add this into your .css file, or as style code into your header.
